Medical Case Manager

30-40 hours per week, Full-Time Medical Case Manager position for Evansville/Southern, Indiana. Mon-Fri. Daily regional travel required on field cases. Laptop, cell phone, home office phone and iPad provided.<br>Essential Job Duties: &nbsp;&nbsp; &nbsp;Responsible for assessment &amp; coordination of medical care for injured workers. Works closely with adjusters, medical providers, employers &amp; attorneys to ensure appropriate care to medically rehabilitate individuals &amp; return them to work. Makes recommendations regarding health care resources. Provides assessment, planning, implementation, and evaluation of patient's progress. Evaluates patient's treatment plan for appropriateness, medical necessity, and cost effectiveness. Attends physician appointments, hospital and/or long-term facility discharge planning conferences, for the purpose of determining appropriateness of care and developing an effective long-term care strategy. Regional travel required on field cases. Reliable transportation required. Requires regular and consistent attendance. Extensive documentation using various computer programs. Communication is frequent and completed by using telephone, email and computer programs.<br>Education/Training Requirements: &nbsp;&nbsp; &nbsp;Education/Training Requirements: Graduate of an accredited school of nursing with an associate’s degree or higher. Registered nurse with current, valid state licensure in good standing. 3+ years clinical nursing experience. Case management, workers' comp experience strongly preferred. EXCELLENT technology skills, organization &amp; documentation skills a must.
